Breg charged us $400 for a $150 boot

Back in June, my husband went to urgent care for severe ankle pain. Ultimately he was diagnosed with gout, but before the blood tests the doctor sent him home with a Breg ankle boot.

We then received a bill from Breg in the mail. They charged our health insurance $400 for the boot, health insurance allowed $195.32 and we owe that amount out of pocket.

However, when I look at this boot online, I can buy it outright for anywhere between $115-$150. Breg’s own store on Amazon has the exact same boot listed for $150. So I don’t understand how Breg can charge $400 for it? I don’t feel we should be paying more out of pocket for a boot just because it went through health insurance, when we could’ve bought it outright online for less.

Do I have a leg to stand on to dispute these charges? If so, do I dispute it through Breg directly or through my health insurance?

The UG code in the notes of the EOB says this: The plan discount shown is your savings for using a network provider. You have not met your deductible and owe the amount shown.
