Reimbursement of visit paid out of pocket?

Basically, my insurance company wasn’t covering a doctor visit due to them not sending enough medical records from a drug screening, this is a monthly visit and there was three visits that weren’t covered and they told me that they had gone back-and-forth with the insurance company and basically I had to pay $600 to cover my visits the insurance did not cover. Well, my insurance company talked with the doctors office accounting and got things squared away, and figured out what documentation needed to be sent to get these visits covered, I see on my app with the insurance that the claims were redone and approved… but the doctors office is claiming the insurance is not going to cover them. I have already paid the money that these visits cost directly to the provider and they are telling me the insurance is still not covering me, but it shows on my app they are? So who do I go to for reimbursement if they really did cover the visits as I am seeing? Or am I just out of luck on the money? This is through Blue Cross Blue Shield . And these visits are with my primary care doctor
