Health insurance charging $8-10k than M.O.P. how can I fight this?

In 2022 I had significant amount of medical expenses. So much so, that I exceeded my families Maximum Out Of Pocket (M.O.P ). Insurance and the hospital are now claiming I owe $8-10k above the $17k which was my M.O.P. I kept detailed records of all my medical expenditures for 2022 because I knew it would be ridiculous. I have talked to insurance and they keep just thinking it might be balance bills but they claim it's not. We knew this was going to be a ordeal so we double-tripple checked as much we physically were able to make sure everything was pre-approved and covered. My plan does not have Maximums to coverage and all doctors and hospitals are "in-network" as long as they follow medi-caid pricing. What should I try to do to resolve this in my favor? 79404 zip code.

TLDR: Insurance/Hospital says I owe $8-10k more than my M.O.P. despite keeping detailed records and keeping everything in network. This is for my newborn (in 2022) son.What can I do to fight this?
