Insurance saying they haven’t received a prior-authorization, Dr confirms its been sent 4 times

I need a prior authorization. My doctor submitted the PA through the insurance companies web portal while I was in the office with them on July 5. My insurance company takes up to 10 business days to process so when I didn’t see anything online after 10 BDs (you can track PAs online) I called. They said they never received it. I called the Dr office who checked their system and confirmed it was sent and showing as pending but they also filled out some forms and faxed it over to the PA department and have fax confirmation it was received July 22.

Faxes take 24-48 hours to process so I called the insurance company the following week and they again said they didn’t receive it. I confirmed the fax number with them and had the Dr’s office re-send it same-day. They again say they didn’t receive it. They then offered to call the Dr’s office and get a verbal PA but after 3 failed attempts during a 2-week period they still were unable to contact the Dr’s office. My Dr again sent the PA through fax Aug 11 (this past Friday) and the insurance company still claims they haven’t received it.

So I don’t know what to do. Any advice?
