Employer-paid PPO insurance or other PPO insurance with premiums?

Hi, all. Wanted to see if anyone had any recommendations for what health insurance to choose. The obvious choice may seem like the employer-paid one, but I do have some health issues and anxieties, so wanted to see if anyone had any input or questions I had not yet considered:

*I take regular prescribed medications that is covered usually

*I sometimes see specialists (dermatologists, OB/GYN specialist, allergist)

*I go to a primary 1-2x/year and try to get STI tested every 4-6 months or so wherever I can (sometimes more often)

*I try to have a good plan where frequent urgent care visits won't run me dry – At one point, I was going to an urgent care almost every month or so (not currently, though)

*I have a lot of health (and financial lol) anxiety and try to also have a good plan in the case I have to go to the ER for whatever reason, especially knowing my random health; I would prefer not to spend a lot at once as I only have <10k in savings that I absolutely do not want to touch

Plan 1: PPO, $0.00 per pay period (employer pays all of it), $6,350 deductible with $6,850 out-of-pocket max, 0% co-insurance after deductible is met for specialist visit, urgent care visit, and ER visit.

Plan 2: PPO, $43 per pay period, $5,000 deductible with $6,850 out-of-pocket max, $80 copay/ specialist visit (deductible doesn't apply), $85 copay/urgent care visit (deductible doesn't apply), $350 copay/ER visit (deductible doesn't apply).

Idk, it seems like Plan 2 seems best in the case I have an emergency or urgent matter come up (esp. after being recently diagnosed with some health conditions) and seems like it'd be cheaper even considering all my premiums.
