Getting charged practice for balances even though my HRA has been fully paying them?

I searched for a similar post and looked through the pinned post, but can’t find a similar situation or what to do.

Basically, I have been going to a practice and seeing two providers there since November. I live in Delaware and work for the state government so I have a pretty excellent plan through Aetna. I have a health reimbursement account where almost my entire deductible amount is covered by this fund. The provider has been billing my insurance and between my plan and the HRA, I can see the entire balance has been covered every time.

The issue is that the practice has still been billing me (a cumulative $500+) and telling me that it’s because I haven’t met my deductible so my insurance isn’t paying them. I confirmed with my insurance this isn’t true. I also got a flexible spending account recently that I used for several visits and now the FSA provider is requesting I pay them back because insurance already covered the visits.

I am at a loss for where to go from here. I don’t really understand my options for how to get reimbursed from this. Could I threaten legal action? Am I just screwed out of this money?
