Final bill seems inaccurate but my therapist was likely committing licensing fraud by practicing out of state via zoom. What can I do?

TLDR: my bill is oddly way higher than what I expected and seems inaccurate. The only issue with bringing this up with my insurance company is that I moved out of state about halfway through services but continued to get therapy via tela-health and I believe my therapist just left my old address on the paperwork (I believe this is not legal). What should I do?

OK so at the time I was on my moms insurance, she got that insurance in like march of last year and in a previous bill I received, the insurance was working as planned and my co-pay was $20. Fast forward to this year, I get a final bill with all of the rest of the sessions on it, and oddly, all of the sessions from 2022 were not covered by insurance, but as soon as 2023 rolled around the co-pay went back down to $20, even though there was no change in insurance, and no yearly deducible was met, ect.

I moved out of state in about September of 2022 and I told my therapist that I wanted to keep seeing her. She said she could make it work and that she was not really suppose to do whatever she did but she would help me out. I believe she just left my old address on all the paperwork. The only issue is that if I make a claim with my insurance or bring this up to insurance at all, they can very easily see that I moved out of state in September. Im worried about the legal ramifications for me and for my therapist. But more importantly, this bills is HUGE and about 6x bigger than it would be if my insurance covered it, and at this point I dont see why my insurance didn't cover the 2022 sessions, but did cover the 2023 sessions.

She is a private practice and uses a third party to do billing so its taken a very long time just to get a bill, after many weeks of asking for it. Please for the love of god give me any advice you have. This is immensely stressful for me.
