Is the event of the change the first or second paycheck?

I finally have an answer to how many hours I will regularly be working. I will be just over the income limit for Medicaid in Washington state. When I called my health insurance at the end of last month ( I started working that week I called) I had no idea what to put for my monthly income. I was told my job can vary from 40 hours one week to 20 another week. Medicaid assured me that It's okay if I wait to get a better understanding of my hours first and then report within 30 days. I will be over my income limit after my second paycheck at the end of this month. So do I report after my second paycheck? Also as I was talking with my Medicaid I told them I have a doctor's appointment (the first one I had since getting Medicaid) at the end of the month they said that is fine but it just feels wrong to me that I had a doctors appointment and then I will be reporting an income that will disquiflfty me from their insurance.

Additional Details: My first real paycheck was May 11th my second one will be the 25th of May. The Doctors appointment was today the 22nd. I will have to get a mammogram soon so I'll rather not have to worry about issues with my health insurance and possibly being charged with fraud at the same time.
