[CO] Anthem cannot locate my second level appeal

Just as the title describes, anthem cannot locate my 2nd appeal on a medical insurance claim. Obviously, my first appeal got denied, and as per anthem policy, you have to physically mail in a grievance letter to request a second level appeal. I did that, got usps certified mail/tracking on it, and addressed it to the address the first appeal denial letter listed. It was delivered March 31.

I called anthem around a week or two ago to request with the status was on the second level appeal, and they told me they couldn’t find it, but would investigate further. Today I received an email saying that they cannot find it. They also said that it needs to be addressed to a DIFFERENT address than the one outlined in the first appeal denial letter (which I still have, and I confirmed that I sent to the address I was instructed to in the letter). I’m now outside the window of being able to request a second level appeal now.

What are my next steps? Do I request a second level appeal again? Do I complain to the state?
