I've been going to a local university dental school clinic and they don't participate with commercial insurance. I have Delta Dental and I've never had any issues getting my submitted claims processed until recently. Delta has started denying claims saying the diagnosis code is invalid.

The school pre-populates the ADA form and they include a diagnosis code in box 34a. Delta is claiming that box 34a must be left blank unless there is a medical diagnosis that requires coordination of benefits. The school says their EHR pre-populates the form and they cannot change the way it gets filled out, and that they have filled out the form this way for many years and plenty of people have Delta insurance–it doesn't make sense that would only now become a problem.

What can be done? It's annoying to have to call Delta customer service every time to have the claim reprocessed.
