What can I talk to doctor about in annual physical without incurring an office visit fee?

I’m going to have that free annual physical/wellness exam next week with my PCP in an HMO. I understand I’m not supposed to ask about stuff that would make it a whole ass different consultation, like I can’t talk about how I want to check if there’s something wrong with my toenails or something, because then it’s a separate consultation and they’ll charge me more.

I’m wondering if there’s anything I can talk about though. Here are things I would bring up:

My thyroid is in the low range of normal, but I have ADHD-like symptoms which could also be a thyroid problem anyway. Can I ask for them to do some thyroid treatment?

I am not sleeping as long as I used to, which may be contributing to ADHD-like symptoms. Should I ask for a sleep study?

I think I might need to be treated for HRT for the same reason, should I mention that?

I want to visit a dermatologist for this thing on my neck. Do I point it out to the doctor?

Or do I just not mention any of this, and then later ask for a referral online/via email/phone?
