Which employer plan should I go with?

I’m probably changing jobs soon and have to figure out which insurance plan to go with. My current employer keeps it simple, one plan for everyone, but the new employer has two options. One seems like an HDHP, the other is like a mix of HMO and PPO it seems. Both have the same Aetna network.

The health center plan costs $100 per month versus zero dollars per month for the consumer choice plan. The consumer choice plan lets you get an HSA while the health center plan lets you get a flex account. Consumer choice lets you pick your PCP, health center requires you to go to some specific facilities that serve employees on that plan. This sounds like a no-brainer right?

Unfortunately I’ve got some ongoing medical issues that will probably need attention throughout the rest of the year at least. That involves some regular checkups with my PCP (who I’ll have to switch with this new job regardless), regular blood work, and checkups with a specialist. Possibly imaging as well (ultrasounds mainly). I’m also considering getting a CPAP as I was diagnosed with mild sleep apnea, presumably separate from the aforementioned issues.

If there’s one thing I can’t stand, it’s unpredictability in what I’m gonna pay when seeing the doctor. You never know what number they’ll pull out of thin air. I think I’d rather just have a consistent co-pay so I can properly budget for it.

Given that, which plan should I go with? The health center plan? My specialist would be in-network so that’s no problem (I think).
