COVID Test lab we used in Feb 2022 seems to be out of network as per new claim we received in Dec 2022, please help!

We (31 F and 31 M couple, in CA) took a RT-PCR test for travel back in Feb 2022 from an independent lab back when traveling internationally needed it. I remember they advertising that they take BCBS, we uploaded our insurance card etc.. Fast forward to December 2022, we got email from insurance that they are out of network and we have some $500 dollars to pay for the two of us. I particularly remember to go to them because they were covered. Now that I check my insurance, it says they received the claim on December 2022, which they mentioned is from an out of network provider as per the recent emails. I don’t find any email from the lab unfortunately to prove they were in network when we booked them. Is it possible they became out of network after February 2022? What are my options here – should I reach out to insurance or to the provider to sort it out and possibly reduce the amount if I cannot get it 100% covered? Please help, thank you to the community!

Edit: Added age and state. Also, another thing – the lab hasn’t reached out to us asking for paying remaining amount. If they doesn’t ask, are we good to leave it as it is and not pay anything? Should we wait for them to send us an invoice to pay the bill before contacting them?
