I received a Notice of Payment Due from a life insurance provider, naming me as the Insured, but no mention of who is the Owner or Beneficiary for this policy. The bill lists an amount due for the Semi-Annual Premium, and an amount for Loan Interest Due.

I’m not aware of any life insurance policy being set up for me, or anyone who would have done such a thing. Is it possible that someone fraudulently set up a policy under my name, in order to borrow money off it?

I called the life insurance company to explain the situation and ask questions, but was told that as the insured I don’t have access to any information about the Owner or Beneficiary, or any amount borrowed. They said to submit my request in writing via email, which I did, but haven’t heard back in a few weeks.

Is there any way to find out more info about this policy? Can something like this affect my credit score, or ability to apply for other insurance, or could it cause problems for my relatives if I were to die?
