Experience getting fitness reimbursement?

My plan offers a fitness reimbursement of up to $150 for “qualifying health and fitness programs.” It’s extremely vague but lists a few categories: health and fitness, wellness (meditation/massage), weight loss, and maybe one other. For fitness they say for some programs you need to prove 4 months of paying at a “qualified or professional health or fitness program” but that’s the only guideline and it’s still vague.

The form to ask for reimbursement only asks your member info and then says to attach an invoice with all the fitness club info/invoices.

I ride horses with a licensed/professional instructor and while $150 barely covers two lessons, anything is better than nothing. But I pay her business via Venmo (her business account) So I put together an invoice as best I could with her letterhead, all the dates and costs of my lessons for 4 months, and Venmo’s transaction statement. I mailed this in like two months ago and haven’t heard anything back.

Anyone have experience redeeming this benefit? Would they deny me or ask follow up questions? Will I just get a check in the mail one day?
